Memorial Scholarships:

  • Two $500 dollar scholarships awarded yearly.
  • One $500 scholarship to a qualified student in an undergraduate/graduate Allied Health program.
  • One $500 scholarship to a student with RSD,CRPS, or a chronic pain condition pursuing postsecondary education.

For students with rsd, crps, and chronic pain conditions:

What you need:

  • One completed and signed application form.
  • One Essay
  • One letter of recommendation from a professor.
  • One Copy of your most recent college transcript/s.
  • One Medical Verification of Disability Form.
  • One verification of enrollment form from your school or the National Student Clearinghouse.

For Allied Health students:

What you need:

  • One completed and signed application form.
  • One Essay.
  • One letter of recommendation from a professor.
  • One Copy of your most recent college transcript/s.
  • One signed verification of enrollment form from your school or the National Student Clearinghouse.

For all applicants:

  • Deadline is December 1, 2012
  • First Awards to be announced in February 2013
  • Winners are selected based on the overall strength of their submission packet.

Mail completed application, essay, letter of reference, copy of your transcripts, and verification of enrollment form to:
